Housing Stabilization Services

 

Housing Stabilization Services (HSS) is a Minnesota Medical Assistance Home & Community Based (non waiver) service benefit to help people with disabilities and seniors find and keep housing.

At Olodhi, we offer Housing Consultation, Housing Transition and Housing Sustaining

​

Housing Consultation: This service develops a person-centered plan for people without MA case management services.

Housing Transition: This service supports a person to find housing.

Housing Sustaining: This service supports a person to maintain housing.

 

How To Qualify

To qualify for HSS, you must meet the following criteria:

​

18 years of age and receiving Medical Assistance

Have a disability or a disabling condition

Be experiencing housing instability

Have a need for services due to limitations caused by individual disability

 

Documents Required

 

Documents needed for DHS Eligibility Verification Review:

​Professional statement of need, OR

Medical opinion form, OR

Proof of recipient of SSI or SSDI

AND

​

MNChoices Assessment, OR

Long term Care Coordination Plan, OR

Coordinated Entry Assessment

Increase long-term stability in housing in the community.
